This is an example of a survivor-driven, trauma informed screening form that was developed by New Beginnings for their Bridge Housing program. The form includes questions to ask a caller that is looking for housing to identify if they are dealing with the effects of domestic violence and if they are a fit for the program/the program is a fit for them. The form also includes helpful information to share with the caller about the program.
